Below are links to bank forms, which you can use to apply for new services, as well as make appropriate changes to authorizations and access to individual products.
PAPER DOCUMENTS
The form should be downloaded and then opened using Adobe Reader.
Completed applications approved with a qualified electronic signature should be sent directly to your CitiService Relationship Manager or to citiservice.polska@citi.com.
Applications signed by hand should be delivered in original to the address:

Authorization for company representatives for daily contact with the bank and receiving confidential information regarding accounts and products.

Account Authorization Update Form
- Updating persons authorized based on resolutions of the management board, powers of attorney, business registers, or other documents authorizing action on behalf of the client for the purpose of opening, closing, and maintaining accounts.
- Updating persons authorized to issue instructions related to crediting or debiting an account.
- Updating persons authorized to sign and authorize payment instructions ordered on paper forms (as well as to change, withdraw, or cancel previous instructions).
- Updating persons authorized to confirm payment instructions ordered on paper forms by phone.
